Blood Transfusion After Hip Fracture Surgery in Elderly Patients

Aim:

To determine blood transfusion in elderly patients with collum femoris fracture and intertrochanteric femur fracture who underwent hemiarthroplasty and to compare the two fracture groups for the amount of blood transfusion.

Methods:

Hemoglobin and hematocrit values were determined preoperatively and in the last control before hospital discharge. T-test for non-parametric Mann-Whitney U test and paired t-test for non-parametric Wilcoxon test were performed for statistical analysis.

Results:

The mean age of patients with fracture of collum femoris and those with femur intertrochanteric fracture were 84.3 years (range: 77-96) and 83.3 years (78-89), respectively., The mean preoperative hemoglobin and hematocrit levels of collum femoris fracture group were 12g/dl (10.5-15.2) and 37.6 (31.1-48.4), respectively. The mean predischarge value for hemoglobin was 11.1g/dl (9.2-12.6) and for hematocrit was 34.2 (29.5-38.8). The mean preoperative hemoglobin value was 11.4g/dl (9.9-13) and hematocrit value was 35.6 (31-41.3) in the intertrochanteric femur fracture group. The mean predischarge value for hemoglobin was 11g/dl (9.5-12.6) and for hematocrit was 33.4 (29.9-38.9). The average number of blood units transfused in collum femoris fracture group and intertrochanteric femur fracture group were 1.7 (0-5) and 1,9 (1-5), respectively.

Conclusion:

There was no statistically significant difference between the groups in amount of blood transfusion. Two units of packed red blood cells should be prepared prior to surgery.
